Before You Arrive to Your First Appointment for Orthotics or Prosthetics

In order to make your first appointment efficient and comfortable, it’s important that you bring the following items:

Insurance card(s)

If you’re covered under multiple insurance plans, please be sure to bring each card

Photo ID

Valid driver’s license

Prescription or referral

*if applicable

How to Prepare for Your First Visit

Make Time

Your first appointment could last up to one hour, so make sure you set aside ample time to discuss your needs, expectations, and lifestyle goals with our practitioner. 

Have Questions

Write down your questions ahead of time to avoid forgetting important topics you’d like to discuss.

Dress Appropriately

Many procedures for advanced prosthetics and custom orthotics require measurements and casting. Wear loose, comfortable clothing and underwear; shorts or a tank top are ideal for arm and leg casting, respectively.

If you haven’t filled out our new patient forms ahead of your appointment, arrive early to allow yourself enough time to complete them.
